2. Paper Work:

Please print and fill out this Service Request Form and sent it to us attached to your media. Please, do not forget to sign it.

3. Packing your media:
Remove your hard drive or digital media. (your local computer store service technician can assist you).

Place your media in an anti-static bag. (If you don't have one, you can obtain one at your local computer store).

Provide ample and tight padding around all sides of your media, suspending the media in the middle of the package. This will help to reduce the risk of your media being damaged in transit.

If you must ship your entire desktop, laptop, RAID tower, etc., please contact us in advance to discuss shipping methods.

Again, please make sure you include the signed Service Request Form; this will ensure prompt and immediate attention upon arrival of your media. No work can be performed, including evaluation, without a signed Service Request Form.
